To participate in our PHAZE Urban Art Program you need to complete a registration form. Please be aware this event is for young people aged 12-25.

To participate in our PHAZE Urban Art Program you need to complete a registration form. Registration only needs to be completed once per season and helps us keep you informed about upcoming PHAZE sessions, events and any changes to scheduled activities.

How to register:

1. Download the PHAZE registration form

2. Complete all required fields (including parent/guardian consent for participants under 18)

3. Attach the completed form to your PHAZE registration form.
