When fully and correctly completed and signed these forms may be:

• Faxed to (08) 9330 9760
• Scanned and emailed as an attachment to vaccare@melville.wa.gov.au
• Mailed or delivered to Vacation Care Programme

When they are received by the VacCare staff (and accompanied by a current and signed enrolment form), we will issue you an invoice for the days of care required.

This invoice is payable on or before the due date:-
1. Over the phone with a Credit Card by calling (08) 9364 0808
2. By person at Melville Recreation Centre during normal opening hours.
3. By return mail to the above address

Please Note – Confirmation of your child’s place will not be made until full payment is received. If payment is not received by the due date the place will not be reserved and may be offered to another family.

Vacation Care enrolments in person will be accepted during the advertised times. These times will be advised approximately 4 weeks prior to the start of the holidays. Late enrolments will be taken only at the discretion of the VacCare Team.

Payment must be made in full at time of enrolment.
