Melville Home and Community Care (HACC) Services provides support to people who choose to live in their own homes but whose age or disability means they need assistance.
What is HACC?
Home and Community Care (HACC) is a joint Commonwealth and State funded program. The program aims to help frail older people and young people with moderate to severe disabilities to be more independent at home and in the community. HACC can enhance quality of life and/or prevent inappropriate admission to long-term residential care.
What do Melville HACC Services offer?
At the City of Melville we offer a small variety of HACC services and can also connect you to a range of other organisations that provide services not offered by us.
The City of Melville provides a range of activities and programs based in a community centre as well as a Meals on Wheels service.
Our community centre based activities include:
- Ethnic Melville Active Seniors (EMAS) - Asian, South Asian & European groups
- Meals to Music program for Aboriginal Elders and their carers
- Teenvac school holiday program for teenagers with a disability.
How can I apply for a service?
Anyone can request a service - yourself, a family member, a friend or a medical provider on your behalf.
A doctor’s referral is not required; however, eligibility criteria need to be met before a service can be offered.
Who is eligible for this service?
When you contact us to request a service an Assessment Officer will arrange a time to meet with you. Basic information about you will be collected during the assessment. This is necessary to determine if you are eligible and to meet your care needs.
The Melville HACC Services Assessment Officer may inform you of a range of services provided by the City of Melville and/or other service providers that will meet your needs. If you agree, these are developed into a care plan which sets out what services you will receive and when.
Will you require an interpreter?
You can request a family member or a friend to assist with interpreting at the assessment, or we will provide an interpreter if you prefer.
What does it cost?
HACC Services are partly subsidised by the HACC Program funding (State and Commonwealth jointly) and the City of Melville. At your assessment you will be provided with information about fees and asked to complete an income assessment form. We will also ask you for a nominal fee towards the cost of providing the service. Contributions vary for each service and are dependent on your income assessment. The fee is reviewed annually along with all your information.
